INTRODUCTION AND PARTIES
These Terms and Conditions ("Agreement") are between you ("client," "I," "me") and DexaFit Greenville LLC, DBA DexaFit Greenville ("DexaFit Greenville," "we," "us"). This Agreement governs in-person services provided at our Greenville, South Carolina location and supplements our website Terms of Service. By booking an appointment, signing at check-in, or accepting electronically, you agree to this Agreement.
INDEPENDENTLY OWNED AND OPERATED
DexaFit Greenville LLC is an independently owned and operated business that licenses the DexaFit trademark, software, and analytics from DexaFit, Inc. DexaFit, Inc. does not own, operate, manage, staff, or supervise this location. All in-person services you purchase at this location are purchased from DexaFit Greenville LLC and are governed by this Agreement.
SERVICES; THIRD-PARTY NUTRITION SERVICES
DexaFit Greenville provides DXA body composition and bone densitometry scans, RMR Metabolic Analysis, and VO2 Max testing.
Nutrition and dietary counseling available at our location is provided exclusively by Second Arrow Nutrition Consulting LLC ("Second Arrow Nutrition"), a separate and independent company that is not owned, operated, or controlled by DexaFit Greenville LLC or DexaFit, Inc. Any nutrition or dietary services are governed solely by Second Arrow Nutrition's own agreements, policies, and privacy practices. DexaFit Greenville LLC assumes no responsibility or liability for any services, advice, recommendations, or outcomes provided by Second Arrow Nutrition, and payment for such services is made directly to Second Arrow Nutrition under its own terms.
If I enroll in, request, or participate in any of Second Arrow Nutrition's services — including any consultation, whether free or paid, virtual or in person — I authorize DexaFit Greenville to share my DexaFit test data and related information with Second Arrow Nutrition to support my nutrition care, including my DEXA scan results, VO2 Max and RMR test results, intake form responses, and data from the DexaFit app. DexaFit Greenville will not share my information with Second Arrow Nutrition unless I enroll in, request, or participate in its services, or otherwise consent.
ELIGIBILITY AND AGE REQUIREMENTS
DEXA body composition scans and RMR Metabolic Analysis are available only to clients 18 years of age or older. No exceptions.
VO2 Max testing is available to clients under 18 years of age only if a parent or legal guardian signs this Agreement (including the Waiver and Agreement and the Informed Consent for VO2 Max Testing) on the minor's behalf and remains present at the facility for the entire appointment.
DEXA bone density (BMD) scans are available to clients 16 years of age or older. Clients aged 16 or 17 require a parent or legal guardian to sign this Agreement on their behalf and to remain present at the facility for the entire appointment.
A parent or legal guardian who signs on behalf of a minor accepts this Agreement, including the Waiver and Agreement, both on the minor's behalf and individually.
DexaFit AI Software: I understand that insights, scores, and reports generated by the DexaFit AI software are not guaranteed to be accurate for clients under 21 years of age, and that for such clients any AI-generated content is provided for general reference only.
USE AND DISCLOSURE OF PROTECTED HEALTH INFORMATION
I consent to allow DexaFit Greenville LLC, DBA DexaFit Greenville, to use their DXA scanner to perform a body composition and/or bone densitometry scan, with full awareness that the technology uses low-dose X-rays. The bone densitometry scan is Tech Only, is not read by a radiologist at DexaFit, and will not be reviewed with the client.
DEXA bone density (BMD) scans, including site-specific scans (such as hip, spine, or forearm), should be evaluated by a licensed physician. DexaFit Greenville does not and cannot comment on, interpret, or evaluate any DEXA BMD scan completed in the facility. Clients are solely responsible for providing their BMD scan results to a licensed physician for evaluation.
RECORDS REVIEW FOR RESEARCH
I also authorize DexaFit Greenville LLC, DBA DexaFit Greenville, to review my records to determine whether my body scan qualifies for approved clinical studies and to contact me if I am a potential candidate for research. No records will be shared with any third parties for research purposes unless I provide specific written approval.
DEXA SCAN ELIGIBILITY AND SAFETY
I acknowledge that DexaFit Greenville's GE Lunar Prodigy DEXA scanner has a manufacturer weight capacity of 350 lbs and cannot accommodate individuals weighing 350 lbs or more.
I certify that I am not pregnant and not attempting to become pregnant, and I agree to inform staff of any change before any scan.
I agree to arrive prepared in accordance with the pre-test instructions provided at booking (including clothing free of metal for DEXA scans). Arriving unprepared may result in cancellation of the test and the reschedule/late cancellation fee described below.
RMR METABOLIC ANALYSIS
I consent to Resting Metabolic Rate (RMR) testing, which measures the caloric requirements of my body at rest through analysis of my breathing. I understand that accurate results require compliance with the pre-test fasting and preparation instructions provided at booking, and that results are informational tools, not medical advice. If a test cannot be performed or must be repeated because pre-test instructions were not followed, the appointment will be treated as a reschedule and the $45 reschedule/late-cancel fee described below will apply.
CLIENT-SUPPLIED EQUIPMENT: BICYCLES AND ROWING MACHINES
Bicycle Testing on the Wahoo KICKR Trainer. DexaFit Greenville does not maintain a stationary bike. VO2 Max cycling tests are performed on the client's own bicycle mounted to the facility's Wahoo KICKR direct-drive smart trainer. Mounting a bicycle to a direct-drive trainer requires removing the rear wheel and attaching the bicycle to the trainer using the bicycle's quick-release skewer or thru-axle, with the chain engaging the cassette installed on the trainer.
Compatibility. I am solely responsible for confirming that my bicycle is compatible with a direct-drive trainer, including frame material and design (including carbon fiber frames), rear axle standard and dropout spacing, drivetrain and cassette speed compatibility, and any manufacturer restrictions or warranty limitations that apply to trainer use of my bicycle. I understand that some bicycle manufacturers limit or exclude warranty coverage for damage arising from stationary trainer use, and that it is my responsibility to know whether such limitations apply to my bicycle.
Equipment Condition on Arrival. I agree to bring my bicycle in a reasonably clean, ride-ready condition — free of mud, dirt, sand, grass, and other debris, and without excessive oil, grease, or degreaser residue on the drivetrain or frame. A dirty or contaminated bicycle can damage the trainer, transfer grit into the trainer's cassette and drivetrain, and soil the testing area. DexaFit Greenville may decline to mount a bicycle that arrives excessively dirty or contaminated, in which case the appointment will be treated as a reschedule and the $45 reschedule/late-cancel fee will apply. The same applies to a rowing machine that arrives in a condition unsuitable for indoor use.
Mounting, Use, and Dismounting. I am responsible for the mounting and dismounting of my bicycle, including removal and reinstallation of the rear wheel. If DexaFit Greenville staff assist with mounting or dismounting as a courtesy, I remain solely responsible for verifying that all work was performed correctly. I understand that riding a bicycle on a trainer at maximal effort places significant loads on the frame, drivetrain, and components.
Assumption of Risk and Release for Equipment Damage. To the fullest extent permitted by law, I assume all risk of, and release DexaFit Greenville LLC, DBA DexaFit Greenville, and DexaFit, Inc., and their representatives from any responsibility or liability for, any damage to, wear of, or mechanical failure of my bicycle or any part or accessory of it — including but not limited to the frame, fork, rear wheel, axle, quick-release skewer or thru-axle, drivetrain (chain, cassette, chainrings, cranks, bottom bracket, derailleurs, and shifters), brakes and brake rotors, and any electronic components — arising out of or in any way connected with transporting the bicycle into or out of the facility, mounting or dismounting the bicycle on the trainer, use of the bicycle during testing, or the condition of the bicycle after testing, whether occurring during my appointment or after I leave the facility, and including damage caused by negligence to the fullest extent such a release is permitted by law.
Mandatory Post-Test Safety Inspection. I understand and agree that BEFORE RIDING MY BICYCLE AFTER TESTING, I am solely responsible for inspecting it and confirming that it is safe to ride, including at a minimum: (1) that the rear wheel is correctly seated and aligned in the dropouts; (2) that the quick-release skewer or thru-axle is installed and tightened to the manufacturer's specified torque; (3) that all brakes are fully functional, including confirming that disc brake pads and pistons were not compressed while the wheel was removed and that the rotor is properly seated in the caliper without rubbing; (4) that shifting and derailleur function are correct; and (5) that all other fasteners and components are secure and at proper torque. If I am unsure how to perform any of these checks, I agree to have my bicycle inspected by a qualified bicycle mechanic before riding it. My decision to ride the bicycle after testing constitutes my confirmation that I have inspected it and accepted its condition, and DexaFit Greenville is not responsible for any injury, death, or property damage arising from the condition of my bicycle after it leaves the facility.
Client-Supplied Rowing Machines. Clients may supply their own rowing machine for VO2 Max testing. I am solely responsible for transporting my rowing machine into and out of the facility, setting it up, and confirming that it is stable, properly assembled, and safe to use. To the fullest extent permitted by law, I assume all risk of, and release DexaFit Greenville LLC, DBA DexaFit Greenville, and DexaFit, Inc. from any responsibility or liability for, any damage to my rowing machine arising out of or connected with its transport, setup, use during testing, storage at the facility, or removal, whether occurring during my appointment or afterward.
Right to Refuse Unsafe Equipment. DexaFit Greenville may, in its sole discretion, decline to mount, test on, or continue testing on any client-supplied bicycle or rowing machine that appears incompatible, damaged, or unsafe. If a scheduled test cannot be performed because client-supplied equipment is incompatible, damaged, or unsafe, the appointment will be treated as a reschedule and the $45 reschedule/late-cancel fee will apply.
FINANCIAL RESPONSIBILITY
I accept full financial responsibility for all charges for services provided to me and/or my family members. I verify that I am the cardholder and authorize DexaFit Greenville LLC, DBA DexaFit Greenville, to charge the card on file in my account for purchases, no-show fees, and late cancellation or rescheduling fees. By providing a payment card at booking or check-in, I consent to the card being securely stored on file and used as described in this section. If DexaFit Greenville LLC, DBA DexaFit Greenville, is unable to charge the card on file, any applicable fees must be paid immediately and may be subject to collections if unpaid. Fees must be paid before any future service appointments will be honored.
No refunds on tests rendered. No refunds or partial refunds on unused tests from purchased packages, except as provided in "Refunds; Processing Fees" below. Packages currently do not expire; however, DexaFit Greenville reserves the right to introduce expiration terms for packages purchased in the future, and any such terms will apply only to packages purchased after those terms are posted. Clients will not receive reminders about unused tests.
Voucher codes, gift cards, or discount codes must be applied at the time of scheduling. If you have issues applying a code, please contact us at (864) 990-4425. If payment has already been made in full, we cannot issue a refund to retroactively apply a voucher, gift card, or discount code. However, these may be used toward your next appointment.
Refunds; Processing Fees: No refunds on tests rendered. No refunds for no-show appointments. If you cancel a booking within seven (7) days of the date of purchase/booking and before services are rendered, you will receive a refund to the original payment method, less any applicable reschedule/late-cancellation fee and less any non-recoverable credit card or payment processing fees (including fees charged by Square) incurred on the original transaction. To cancel for a refund within this 7-day window, you must call (864) 990-4425 and speak with an agent; cancellations by voicemail, text, email, or online reschedule do not qualify for a refund. Beginning on the eighth (8th) day after purchase/booking, all sales are final and no refunds will be issued; any exception is granted solely at the discretion of DexaFit Greenville management and, if granted, is likewise issued net of such processing fees.
We accept HSA/FSA cards, but it is the client's responsibility to verify coverage. We can provide an itemized receipt upon request for reimbursement. I understand that HSA/FSA eligibility is plan-specific and may require a Letter of Medical Necessity from my physician, and that the reimbursement process is my responsibility, not DexaFit Greenville's.
FACILITY POLICY
Well-behaved children are welcome at your appointment provided they are monitored by an accompanying adult at all times. If a child becomes disruptive or cannot be safely supervised, we may ask that the child leave the testing area, and if necessary the appointment may be canceled or rescheduled and a late cancellation fee will apply. A parent or legal guardian required to accompany a minor client under "Eligibility and Age Requirements" must attend and remain present for the entire appointment. For radiation safety, staff may ask children and companions to remain in the waiting area while a scan is in progress.
Pets are not permitted in the facility, with the exception of trained service animals as defined by the Americans with Disabilities Act (ADA).
To protect our testing equipment, please arrive free of cologne, perfume, body oils, and lotions. These products transfer onto the scanner table, sensors, and testing masks, can be difficult to remove, and may damage equipment or affect the accuracy of your results. If necessary, staff may ask you to remove these products before testing, or reschedule the appointment, in which case a late cancellation fee may apply.
If two clients are scheduled back-to-back, they may enter together.
CANCELLATION / LATE ARRIVAL POLICY
DexaFit Greenville is an appointment-based office. You may reschedule your appointment yourself online, free of charge, as many times as needed, provided each change is made at least 48 hours before your scheduled service time, using the reschedule link in your booking confirmation; the link expires 48 hours before your appointment. Free rescheduling remains available even after the 7-day refund window has passed — the 7-day window applies only to cancellations for a refund, not to rescheduling. If you are unable to attend your appointment, please notify us at least 48 hours in advance so that others may utilize the time slot. Arrivals more than 10 minutes late may result in rescheduling to avoid disruption, and a late cancellation fee will apply. If you are more than 10 minutes late without notifying DexaFit Greenville, your appointment will be canceled. If you arrive unprepared for a test or decide not to proceed with a scheduled test, a late cancel fee will apply for that test.
Reschedule or Late Cancellation Fee: If you cancel or reschedule within 48 hours of your scheduled service time, you will be charged $45 per test scheduled, regardless of when the appointment was booked. For prepaid appointments or packages: the $45 reschedule/late cancel fee will be deducted from the prepayment. The remaining credit can be used toward your next visit, or the missed test will be deducted from the package. No partial refunds.
No-Show Policy: If you fail to show up for your appointment without canceling, there are no refunds. Prepaid amounts for no-show appointments are forfeited, and no-show tests will be deducted from purchased packages. For appointments that were not prepaid, a $45 no-show fee per test scheduled will be charged to the card on file.
WAIVER AND AGREEMENT
I hereby release all representatives of DexaFit Greenville LLC, DBA DexaFit Greenville, and/or DexaFit, Inc., including those acting on their behalf, from any responsibility or liability for injury or damages to myself, including those caused by negligence or omission, arising out of participation in any services or programs.
I am voluntarily participating in services provided by DexaFit Greenville LLC, DBA DexaFit Greenville, and/or DexaFit, Inc., including but not limited to DXA scans, RMR Metabolic Analysis, and VO2 Max testing. I assume all risks of injury or death resulting from these services.
I declare that I am physically sound and have no condition that prevents me from receiving services from DexaFit Greenville LLC, DBA DexaFit Greenville, and/or DexaFit, Inc. I confirm that I have either obtained my physician's approval or assume responsibility for proceeding without it. I also certify that I am not pregnant or attempting to become pregnant.
I accept full responsibility for any actions I take after receiving services and do not hold DexaFit Greenville LLC, DBA DexaFit Greenville, or DexaFit, Inc. responsible for any adverse effects or complications.
I understand that data obtained during my scan(s) and analysis is considered confidential and will be treated as such. It may only be used for statistical or scientific purposes, with my privacy rights fully retained.
I understand that DexaFit Greenville LLC, DBA DexaFit Greenville, and/or DexaFit, Inc. does not diagnose or interpret DXA scan results. Any follow-up analysis should be reviewed with my primary care physician.
I understand that DexaFit Greenville is not a medical facility, that no physician is present on site, and that its services are provided for general wellness and educational purposes only and are not intended to diagnose, treat, cure, or prevent any condition. In a medical emergency, call 911.
This release and waiver applies to the fullest extent permitted by applicable law. Nothing in this Agreement waives or limits any claim or right that cannot be waived or limited under applicable law.
INFORMED CONSENT FOR VO2 MAX TESTING
1. Purpose and Explanation of the Test: This is a VO2 Max test. It is NOT a stress test such as you would receive in a hospital or medical setting. This is a maximal exercise test designed to push me to my VO2 Max. I will perform a graded exercise test on a motorized treadmill, on my own bicycle mounted on the facility's Wahoo KICKR direct-drive smart trainer, or on a rowing machine I supply. The exercise intensity will begin at a low level and advance in stages. I may stop the test on my own at any time — by grabbing the arm rails on the treadmill and stepping off safely to the sides, by stopping pedaling on the bike, or by stopping rowing. Although staff will encourage me to continue in order to reach my VO2 Max, I may end the test at any time if I feel dizzy, experience chest pain, or notice any other symptoms that do not feel normal to me. I will wear a chest strap to measure heart rate for training zones, but I understand that staff and the facility are NOT monitoring my heart, are not performing any medical monitoring, and do not know my personal maximum heart rate. If I test on my own bicycle or rowing machine, the Client-Supplied Equipment section of this Agreement also applies.
2. Attendant Risks and Discomforts: As with any exercise, there exists the possibility of certain changes occurring during the test, including abnormal blood pressure, fainting, and irregular, fast, or slow heart rhythm, and, in rare instances, heart attack, stroke, or death. I understand that a physician or licensed medical staff will NOT be present on site. If a life-threatening situation occurs, 911 will be called, but the speed at which emergency help arrives cannot be guaranteed.
3. My Responsibility: I and my own doctor should evaluate the information I possess about my health status or previous experience with exercise-related or heart-related symptoms (such as shortness of breath with low-level activity, or pain, pressure, tightness, or heaviness in the chest, neck, jaw, back, and/or arms) that may affect the safety of my test. My prompt reporting of these and any other unusual feelings during the test is of great importance. I am responsible for consulting with my own doctor before taking the test.
4. Inquiries: Any questions about the test procedures are encouraged before the test.
I hereby voluntarily consent to engage in an exercise test to determine my exercise capacity. I have read this section, understand the test procedures and the attendant risks and discomforts, understand that no supervising physician or licensed medical staff will be on site, and, having had the opportunity to ask questions, consent to participate in the test.
CLIENT HIPAA CONSENT FORM
I understand that I have rights under the Health Insurance Portability and Accountability Act of 1996 (HIPAA) regarding my protected health information. By signing this consent, I authorize DexaFit Greenville LLC, DBA DexaFit Greenville, and/or DexaFit, Inc. to use and disclose my health information for:
- Treatment (including coordination with other healthcare providers),
- Obtaining payment from third-party payers (e.g., insurance companies),
- Operational purposes of the practice.
I understand I may request restrictions on how my information is used. While DexaFit is not required to agree to my requested restrictions, if it does, it will be bound by that agreement. I understand that I may revoke this consent in writing at any time, but that any prior uses or disclosures are not affected by the revocation.
AUTHORIZATION FOR RELEASE OF HEALTH INFORMATION
I authorize DexaFit Greenville LLC, DBA DexaFit Greenville, and/or DexaFit, Inc. to release my health and fitness information to myself or parties I authorize via email, fax, mail, or through the private login on the DexaFit website. I understand this authorization may be revoked at any time in writing by contacting greenville@dexafit.com, except where actions have already been taken based on the current authorization. This authorization remains valid unless revoked. DexaFit Greenville LLC, DBA DexaFit Greenville, and/or DexaFit, Inc. will not share my information without this authorization.
Referred Clients: If I was referred to DexaFit Greenville for testing by a physician, medical practice (such as PartnerMD), or personal trainer, I authorize DexaFit Greenville to share my test results (including DEXA scan, VO2 Max, and RMR results) with the referring physician, practice, or trainer to support my care and training. I understand that I may opt out of this sharing at any time — before or after my appointment — by calling (864) 990-4425 or emailing greenville@dexafit.com, and that opting out will not affect the services I receive.
DISPUTE RESOLUTION; LIMITATION OF LIABILITY
Informal Resolution First. Before filing any claim, I agree to contact DexaFit Greenville at greenville@dexafit.com describing the dispute and to allow thirty (30) days for the parties to attempt to resolve it informally. Either party may bring a qualifying claim in small claims court.
Individual Capacity; No Class Actions. To the fullest extent permitted by law, any claim may be brought only in my individual capacity, and not as a plaintiff or class member in any class, collective, or representative proceeding.
Limitation of Liability. To the fullest extent permitted by law, DexaFit Greenville's total aggregate liability for all claims arising out of or relating to this Agreement or the services shall not exceed the amounts I paid to DexaFit Greenville in the twelve (12) months preceding the event giving rise to the claim. This limitation does not apply to any liability that cannot be limited under applicable law.
GENERAL PROVISIONS
Electronic Acceptance. Acceptance of this Agreement electronically (including at online booking or on an in-office device) has the same effect as a handwritten signature.
Governing Law. This Agreement is governed by the laws of the State of South Carolina, without regard to conflict-of-law principles.
Severability. If any provision of this Agreement is held invalid or unenforceable, the remaining provisions remain in full force and effect.
Updates. We may update this Agreement from time to time by posting a revised version with a new "Last Updated" date. The version in effect on the date of your appointment applies to that appointment.
