INTRODUCTION

DexaFit Greenville LLC, DBA DexaFit Greenville ("DexaFit Greenville," "we," "us"), respects your privacy and is committed to protecting your personal information. This Privacy Policy explains how we collect, use, share, and protect information when you visit our website, book an appointment, buy or redeem a gift card, or receive services at our Greenville, South Carolina location.

DexaFit Greenville is an independently owned and operated business that licenses the DexaFit trademark and software from DexaFit, Inc. Information you provide through the DexaFit mobile applications and software platform (including DexaFit AI) is collected and handled by DexaFit, Inc. under its own Privacy Policy, available at https://www.dexafit.com/privacy. This Privacy Policy covers information collected by DexaFit Greenville directly.

INFORMATION WE COLLECT

Contact and Account Information. When you book an appointment or contact us, we collect information such as your name, email address, phone number, and appointment details.

Gift Card Information. When you buy a gift card, we collect your name and email address, the recipient's name and email address, the delivery date you choose, and any personal message you include. If you give us a recipient's information, you confirm that you are permitted to share it with us for the purpose of delivering the gift card. We use this information to deliver the card, process redemptions, track remaining balances, and answer questions about the card. When a gift card is redeemed, we record the code, the amount applied, and the remaining balance.

Payment Information. Payments are processed by third-party payment processors, including Square and the payments tools of our LeadConnector booking platform (a HighLevel service), which are used for online bookings and gift card purchases. We do not store your full card number on our own systems; payment card data is collected and stored by those processors in accordance with their own privacy policies and security standards. We retain records of your transactions (amounts, dates, services purchased, and gift card activity).

Health and Wellness Information. To provide our services safely and accurately, we collect information such as your age, height, weight, relevant health history and screening responses (including pregnancy status), and the results of your tests, including DEXA body composition scans, DEXA bone density scans, RMR Metabolic Analysis, VO2 Max testing, and grip strength testing.

Communications. We keep records of your communications with us, including emails, texts, phone messages, and feedback.

Website Usage Information. Our website is hosted on Squarespace, which uses cookies and similar technologies to operate the site and provide basic analytics (such as pages visited, device and browser type, and general location). We also use Google Tag Manager and Google Analytics, and we may use advertising tools from Google and Meta (Facebook and Instagram), which use cookies and similar technologies to measure the performance of our advertising and to show you relevant ads. When you arrive from an advertisement, we record the campaign details in the link you followed (such as campaign tags and ad click identifiers) and store them in a cookie and in your browser's local storage for up to 90 days, so we can pass them to our booking platform when you book and measure which advertising works. Some pages also include embedded content from third parties, such as Google Maps and a Google reviews display, which may set their own cookies. You can manage or delete cookies and local storage through your browser settings, and you can limit ad personalization through your Google and Meta account settings.

HOW WE USE YOUR INFORMATION

We use the information we collect to: schedule and manage your appointments; perform your tests and deliver your results; deliver, redeem, and support gift cards; process payments, fees, and refunds in accordance with our Terms and Conditions; communicate with you about your appointments, results, gift cards, and account; respond to your questions and requests; measure the performance of our website and advertising; maintain the safety, security, and legal compliance of our business; and, with your consent, send you marketing communications.

We may also review your records to determine whether your scan qualifies for approved clinical studies and to contact you if you are a potential research candidate, as described in our Terms and Conditions. No records will be shared with any third parties for research purposes unless you provide specific written approval.

HOW WE SHARE YOUR INFORMATION

We do not sell your personal information. We share information only in the following circumstances:

With DexaFit, Inc. Your test data is processed through the DexaFit software platform to generate your reports and app experience. DexaFit, Inc.'s handling of that data is described in its Privacy Policy at https://www.dexafit.com/privacy.

With Service Providers. We use third-party providers to operate our business, including the LeadConnector booking platform (a HighLevel service, used for scheduling, client records, gift cards, payments, and appointment communications), Square (payment processing), Squarespace (website hosting), Google (analytics, maps, and advertising measurement), and Meta (advertising measurement). These providers receive only the information needed to perform their services and are subject to their own privacy and security obligations.

Gift Card Recipients and Purchasers. When you buy a gift card, we send the card, your name, and any message you include to the recipient you name. We may confirm to a purchaser whether a card was delivered, but we do not share a recipient's test results or appointment details with the purchaser.

MD Review Physician. If you purchase the MD Review option with a DEXA bone density scan, we share your bone density scan and the information needed to complete that review with the independent, board-certified physician who performs it through the DexaStrong program. That physician uses your information to prepare your written report and consultation and is subject to his or her own professional and legal obligations.

At Your Direction. We release your health and fitness information to you or to parties you authorize (for example, your physician), as described in the Authorization for Release of Health Information in our Terms and Conditions.

Referring Physicians, Practices, and Trainers. If you were referred to DexaFit Greenville for testing by a physician, medical practice (such as PartnerMD), or personal trainer, you authorize us to share your test results (including DEXA scan, VO2 Max, and RMR results) with the referring physician, practice, or trainer to support your care and training. If you do not want your results shared with your referring provider or trainer, you may opt out at any time — before or after your appointment — by calling (864) 990-4425 or emailing greenville@dexafit.com. Opting out will not affect the services you receive from us.

Team and Group Testing. If you are tested as part of a team, school, employer, or other group program (for example, a university athletic team), we share your results with the organization that arranged and paid for the testing, in the form agreed with that organization. We will tell you at the time of your scan if your results will be shared this way.

Legal Requirements. We may disclose information when required by law, court order, or governmental request, or to protect the rights, safety, or property of our clients, staff, or business.

Business Transfers. If our business is sold, merged, or reorganized, client information may be transferred as part of that transaction, subject to applicable law.

Second Arrow Nutrition. Nutrition and dietary counseling at our location is provided by Second Arrow Nutrition Consulting LLC ("Second Arrow Nutrition"), a separate and independent company owned by Alex Winnicki, MS, RDN, LDN, CSSD, a licensed and board-certified sports dietitian. If you enroll in, request, or participate in any of Second Arrow Nutrition's services — including any consultation, whether free or paid, virtual or in person — you authorize us to share your DexaFit test data and related information with Second Arrow Nutrition to support your nutrition care, including your DEXA scan results, VO2 Max and RMR test results, intake form responses, and data from the DexaFit app. We do not share your information with Second Arrow Nutrition unless you enroll in, request, or participate in its services, or otherwise consent. Second Arrow Nutrition's use of your information is governed by its own agreements and privacy practices.

Mobile and Text Messaging Information. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All of the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

MARKETING COMMUNICATIONS

Marketing consent is not a condition of purchasing any service or gift card. We do not add gift card recipients to our marketing lists unless they book with us or otherwise consent. You may opt out of marketing emails by clicking "unsubscribe" in any marketing email, and out of marketing texts by replying STOP to any marketing text. Message and data rates may apply. Opting out of marketing does not affect service-related communications such as appointment confirmations, reminders, and gift card delivery.

DATA SECURITY

We use reasonable administrative, technical, and physical safeguards to protect your information, including access controls and reliance on established third-party platforms (Squarespace, LeadConnector, Square, and the DexaFit platform) that maintain industry-standard security measures. No method of transmission or storage is completely secure, and we cannot guarantee absolute security. If a data breach affects your personal information, we will notify you as required by applicable law.

DATA RETENTION

We retain personal information for as long as needed to provide our services and for a reasonable period afterward to comply with legal, tax, and record-keeping obligations, resolve disputes, and enforce our agreements. Gift card records are kept for as long as the card has a balance and for a reasonable period afterward. Test results and related health information are retained in accordance with applicable legal and professional standards. You may request deletion of your information as described below, subject to records we are required to keep.

YOUR CHOICES AND RIGHTS

You may contact us at any time to: request a copy of the personal information we hold about you; request correction of inaccurate information; request deletion of your information (subject to legal retention requirements); request the transfer of your personal information to a third party where feasible; withdraw a consent or authorization you previously provided, as described in our Terms and Conditions; or ask questions about our privacy practices. We will respond to requests within a reasonable time and will not discriminate against you for exercising your privacy rights. We may need to verify your identity before fulfilling a request.

CHILDREN'S PRIVACY

Our website and services are intended for adults. As described in our Terms and Conditions, DEXA body composition scans and RMR Metabolic Analysis are available only to clients 18 and older; VO2 Max testing is available to clients under 18, and DEXA bone density scans to clients aged 16 or 17, only with a parent or legal guardian's signature and attendance for the entire appointment. We collect personal information about minors only in connection with those services and with parental or guardian consent. If you believe we have collected information from a minor without appropriate consent, please contact us and we will delete it.

HEALTH INFORMATION

Health information we collect in connection with your appointments is treated as confidential and is used and disclosed as described in this Privacy Policy and in the HIPAA Consent and Authorization for Release of Health Information sections of our Terms and Conditions. Test data may be used in de-identified form for statistical or scientific purposes, with your privacy rights retained.

CHANGES TO THIS POLICY

We may update this Privacy Policy from time to time by posting a revised version with a new "Last Updated" date. We encourage you to review this page periodically.
